Residential Rat Control Rebate Program Application Form

 

The City is pleased to offer a rebate of up to $200 or 50% within the current calendar year to St. Catharines property owners to assist with the cost of hiring licenced pest control organizations to assist with removing rats from outside their residential property.

If you are completing this form you MUST:

  1. Before applying, please ensure you do not have any of the following on your property:
    -  Bird feeders
    -  Vegetable gardens
    -  Dog feces
    -  Waste
  2. Have previously contacted the City and had a by-law enforcement officer inspect your property.
  3. Initiated the recommendations put forward by the City's by-law enforcement officer to attempt to naturally remove the rats from your property.
  4. Hired a licenced Ontario pest control organization to assist with removing rats from outside your residential property.
  5. Have a signed and completed Contractor Rebate Form from the pest control organization that performed the service to remove rats from outside your residential property. You'll be asked to upload the signed form as part of the application process.
  6. Have copies of the receipts for the service(s) performed by the contractor. You'll be asked to upload your receipts as part of the application process (proof of payment in the form of a cancelled cheque, front and back, or by credit card or bank statement with personal information concealed) as part of the rebate process.

Terms and conditions:

In consideration of receiving the rebate under this Residential Rat Control Rebate Program, the undersigned Applicant acknowledges and agrees:  

  1. THAT The Corporation of the City of St. Catharines (City of St. Catharines) is not responsible for the contractor selection, or for the application or operation of the rat control contractor products or services;
  2. THAT for the purposes of this rebate, the property owners must have retained a licensed Ontario exterminator with a valid Ontario Ministry of the Environment and Climate Change (MOECC) licence authorizing pesticide use according to the terms and conditions of the licence;
  3. THAT the City of St. Catharines does not guarantee or warrant the pest control products or services used, the performance, freedom from defects, quality of workmanship or suitability of the products thereof for any purpose, or the suitability of the premises for application. Further, the Applicant acknowledges and agrees to assume any and all additional costs of the services performed or any ongoing rat control services beyond the rebate amount;
  4. THAT the rebate is not applicable to pest control services rendered to the interior of the buildings and is applicable to the exterior of the buildings only;
  5. TO be eligible for a rebate, you must first contact the City and have the exterior of your residential property inspected by a City By-law Enforcement Officer;
  6. THAT a rebate will not be approved without first attempting to remove rats by implementing the tactics suggested by a City By-law Enforcement Officer;
  7. THAT there are no outstanding Property Standards order(s) or waste issues against the applicant property;
  8. TO indemnify, save and hold harmless the City of St. Catharines, and its agents, officials, officers, councillors and employees (collectively, the agents) against all liability, loss, costs, damages, and expenses, causes of action, actions, claims, demands, lawsuits and other proceedings, by whomever made, sustained, brought or prosecuted, including by third parties, involving bodily injury, death, personal injury, and property damage, in any way based upon, occasioned by or attributable to the Applicant’s participation in this Program, including any negligence on the part of the City of St. Catharines, or its agents;
  9. TO refund such rebate to the City of St. Catharines on request, if this application contains any misstatement or misrepresentation on such Applicant’s behalf, or if the Applicant breaches any of these terms or conditions;
  10. THAT all other instructions and program requirements found in this application form must be adhered to by the applicant in order to quality for the rebate.

Additional Instructions and Program Requirements:

  1. Once completed, all applications must be completed and submitted through this form OR handed in to Planning and Building Services (PBS) at City Hall, 50 Church Street, PO Box 3012, St. Catharines, ON L2R 7C2.
  2. All applications must be accompanied by a copy of the original receipt/bill of service from a licenced Ontario exterminator, including proof that the services were rendered to the exterior of the residential property of the applicant (i.e. billing address listed on the bill of service. You will be required to submit receipts from your contractor (proof of payment in the form of a cancelled cheque, front and back, or by credit card or bank statement with personal information concealed) as part of the rebate process. 
  3. All applications must be accompanied by the completed Contractor Rebate Form. Failure to comply with professional recommendations may result in denial of future rebate requests.
  4. Applications do not guarantee a rebate as they are evaluated and processed on a first come first serve basis.
  5. Rebates will only be credited to the property owner for the address where the services were performed. Proof of an active property tax account at the subject address may be required (i.e. a current property tax or water bill).
  6. Applications will only be received for purchases within the current calendar year and must be submitted by the end of the current calendar year. Only one rebate per property will be accepted per calendar year.
  7. Eligible rebates are equal to 50% of the cost to a maximum limit of $200.00 per application. All additional costs are the sole responsibility of the property owner.
  8. A rebate may be obtained for the following residential properties only: a single detached dwelling, a semi-detached dwelling or duplex, and a triplex created from the separation of a single detached dwelling into three separate living units, or a semi-detached dwelling. Business, Commercial, industrial, manufacturing or residential properties with buildings greater than four units are not accepted. Dwellings of any type subject to a Condominium Agreement are not included.
  9. The rebate is not valid for any service inside of buildings or crawl spaces.
  10. The City of St. Catharines actively encourages residents to retain the services of a local licensed pest control service provider, however it is not mandatory to do so. This rebate is for the services provided by a licensed Ontario exterminator only. It does not include pest control products purchased by the property owner at retail outlets for their own application.

Notice of Collection
The personal information on this form is collection pursuant to the Municipal Act, Section 11. This information will be used for the purposes of participation in, and administration of the City of St. Catharines Residential Rat Control Rebate Program, and updates about rat control initiatives.
